"Grab & Go" Document Book - Assignment for Aug. 19th PL Meeting

Bring your document done or a work in progress to the next Provident Living Meeting - Tuesday, August 19th at 10:00 a.m. in the Primary Room at the Stake Center.

If you should have to evacuate your home for any reason, consider putting together the following documents in one place to grab and go. Original documents should be kept in a safe, secure spot, copies of these documents can be of great importance. (This also could be downloaded to a CD or thumb drive.)

Personal:
Cash – if there is no electricity, there is no way to get to your money.
Bank, phone #, account numbers; Tax Returns (1st 2 pages of last 5 years),
Copies of birth certificates, passports. List of properties, Power of Attorney, Trust Agreements, Leases, Rental agreements, Mortgage and car loans,
Resumes and copies of college transcripts. If you have to relocate and need to a new job.
Military records/Medical documents. You will need immunization records for your children to relocate to another school. Also, any medical history that is important to your family.
Marriage license/Copies of your wills, powers of attorney and medical directives.
Immunization records, Drug prescriptions, Eyeglass prescriptions,
Pre-arranged funeral home or cemetery agreements; a record of the locations of originals. Religious certificates - Genealogical records, Church membership record, journal.

Insurance:
Insurance agent, phone #, policy numbers.
Proof you own your property. If your home is leveled, you’ll need proof of ownership to file a claim. Remember, the same thing that leveled your home, will probably have leveled the courthouse where records are kept.
Copy of your deed/Recent property tax bill/Declarations page. Front page of your insurance policy. This has information on it for you to know what insurance you have and who to contact.
Contents of your home. Many people like to videotape the contents of each room to document what they own. Also, making a list of the items in each room, works too. Update every 6 months.

Other Documents:
A current credit-report. This document will list your creditors and account numbers along with their addresses, you are still expected to pay your bills even in the event of an emergency.
Emergency Contact Phone Numbers (local and long distance)
Evacuation Plans – exact locations; Plan A and Plan B
Personal information: Home phone, cell phone, email address, etc. (we forget under pressure)
For each family member:

Full Name: Birth Date: Marriage Date: Height, Weight, Hair color and length, Eye color
Identifying features: (birth marks, tattoos, facial hair, rings, scars, piercings, etc.)
Special Medical traits: Allergies, Handicap, diabetes, heart condition, pacemaker, hearing aid,
